Financial Assistance Walk-in Counseling at UCS Healthcare

UCS Healthcare is offering financial assistance counseling to assist patients in receiving grants to pay for healthcare treatment. From Monday, March 28 - Saturday, April 2, UCS Healthcare representatives will work with individuals on a walk-in basis to determine if you are eligible and to access the grants to help you with existing balances or to pay for future care.

UCS Healthcare, 1300 Woodland Avenue, West Des Moines

DATE/TIME:

Monday, March 28 - Friday, April 1 from 6:00 a.m. - 4:00 p.m. and Saturday, April 2 from 8:00 a.m. - 1:00 p.m.

Who is eligible?

Grant funds are available for persons who need mental health treatment, substance use disorder treatment, gambling use disorder treatment, or medication assisted treatment. If you are struggling to pay your balance, if you don't have insurance, or if you have a high co-pay, co-insurance or deductible, you may qualify for assistance.

How do I receive assistance?

>Bring two paycheck stubs or current tax forms (2020 or 2021)
>Unemployed? Bring a white sheet from Iowa Workforce Development.
>The amount of assistance can depend on your diagnosis, annual income, familysize, insurance or lack of insurance and balance.

Don't assume your income is too high!

>If you think your income is too high, bring copies of current bills -mortgage/rent, car payment/insurance, utilities, other bills. If these aren't in your name, have the person on the account sign & date the bill with the amount you pay & how often.

Grant Eligibility Guidelines

Integrated Provider Network (IPN) Grant

>Any substance use or gambling disorder
>Income guidelines - equal or less than 200% of income poverty guidelines which is $27,180 annually
>No medical insurance
>Underinsured - your insurance plan doesn't cover services (an exception needs to be filed and approved)
>If your insurance deductible is $1,000 or more (an exception needs to be filed and approved)
>We can assist with balances dating back to June 2020 (an exception needs to filed and approved

State Opioid Response (SOR) Grant

>Not eligible for IPN Grant due to income or incarceration
>Opioid or stimulant use disorder diagnosis
>There are no income guidelines but there must be proof of insurance or income on file.

Iowa COVID-19 Grant

>Not eligible for IPN Grant due to income or incarceration
>Patient has mental health or substance use disorder diagnosis
>There are no income guidelines. No proof of insurance is needed.
>If patient has insurance, the grant can cover co-pays, co-insurance and deductible.
>If patient is underinsured and insurance plan doesn't cover services -patient must be receiving at least two clinical services per month.
