Outpatient Substance Abuse Treatment

Outpatient Services

ADMISSION: Prior to admission into the outpatient program, clients must complete an intake or an assessment.  An assessment can be done at UCS on a walk-in basis or another assessment agency of your choice.  The intake must be done UCS.

NO APPOINTMENT NECESSARY

Walk-In Hours:  Monday thru Friday, 8:00 a.m. - 6:00 p.m.

                        Saturday & Sunday, 8:00 a.m. -  NOON

Please bring:

  • Photo identification (Driver's License, State ID, etc.)
  • Proof of Income (Most recent pay stub, last year's taxes, Wage Verification Form "White Sheet" from Workforce Development, etc.)

If the client was assessed at another agency, the referral must be received by UCS by the time of intake.

For evaluations, clients are required to pay prior to services rendered.

COST: Fees for services are based on a Sliding Fee Scale and determined by client income unless covered by Medicaid or other third party insurance.

*Any previous balances of intakes or assessments must be paid in full before client can receive further services.

Intensive Outpatient (Level II.I)

Treatment Groups (4 days per week)   *Individual sessions as needed
Clients may choose to attend morning, afternoon or evening group sessions.  Weekends available

Extended Outpatient (Level I.0)

Treatment Groups (2 days per week)   *Individual sessions as needed
Clients may choose to attend morning, afternoon or evening group sessions.  Weekends available

Group Schedule

Groups are offered:

    Weekdays:  Morning, Afternoon and Evenings

    Weekends:  Saturday and Sunday groups are available

Treatment Curriculum is used to help guide outpatient clients through the program successfully and consists of: Stages of Change, Relapse, Life after Treatment, Biomedical Health Management, Emotional Well Being, Spirituality, What is addiction, Family, Relationships, Society and Community, Employment/Education/Finances and Goal Setting and Problem Solving.  United Community Services follows the ASAM (American Society of Addiction Medicine) criteria.

Successful Completion is based on client and counselor determination of goal-directed progress, attendance, and commitment to change.

No discharge verification paperwork will be released until client outstanding balance has been settled.
