Where are you located?
The Des Moines office is located at 401 SW 8th Street. Located just south of MLK at the corner of Tuttle and SW 8th Street
The Knoxville office is located at 813 N Lincoln, Suite #15 in Knoxville. Located in the strip mall North of the HyVee.
The Ankeny office is located at 406 SW School Street, Suite # 219 in Ankeny. Located in the Neveln Center Building.
What do I need to bring with me when I come into your office for the first time?
New clients will need to have a photo ID and proof of income. Proof of income can be in the form of an insurance card, Medicaid/Title 19 card, Pay Stub or a “White Sheet” from Workforce Development. White sheets are only given out at Workforce Development on Thursdays.
How many groups will I need to attend?
Each treatment plan is individualized for your needs, so there is no way for us to determine how often or how long you will need to attend.
How much will I have to pay?
Payment is determined on many different levels.
Private Insurance – your private insurance company will need to tell you how much they will cover and how much you will be responsible for paying for any services you receive at UCS.
Medicaid – the majority of Medicaid clients will not have to pay any out of pocket expenses.
No Private Insurance/Medicaid? - UCS will review your current income based on the paystub or white sheet that you provide and how many dependents you have and determine what percentage of our total charges you will have to pay.
